Village Physical Therapy contracts with most major health insurance carriers. We are out of network for Blue Local, Blue Home, and Blue High Performance plans. Before scheduling your appointment, we strongly recommend you contact your health insurance company to verify that Village Physical Therapy is in your network and to get a quote of your benefits for outpatient physical therapy in an office setting. Your insurance company can help you determine applicable copayments, co-insurances, or deductibles that may apply to you.
We discuss with you, to the best of our ability, your patient liability after we verify your insurance, as well. Quoted information that we receive from your insurance carrier is not a guarantee of payment. Ultimately, you are responsible for costs as determined by your contract with your insurance carrier.
We are able to see Medicaid beneficiaries under the age of 21.
We offer a prompt payment alternative if you choose not to file with your health insurance.
Direct Access
Direct Access means the removal of the physician referral mandated by state law to access physical therapist services for evaluation and treatment. Every state, the District of Columbia, and the US Virgin Islands allow for evaluation and some form of treatment without physician referral.
